Advance care planning and supported decision-making

Overview

This presentation draws from ongoing work in community-based interventions across the psychology/allied health and primary care space, focusing on older adults including those with chronic disease and/or emerging neurodegenerative conditions/cognitive impairment. I lead a clinical trial called the EARLI project, which aims to tailor a strength-based and identity sustaining approach to advance care planning for older adults receiving home or community care. In this study my colleagues and I are testing the role of reminiscence-based life story work as a way of facilitating and supporting the advance care planning process. We are studying whether life story work can activate meaning-based coping, which is thought to assist older adults in coping adaptively with the experience of living with disability and/or life-limiting conditions. I am also involved in training and policy advice relating to the emerging practice of supported decision-making, which is increasing in prominence due to its role within the incoming Aged Care Act. This has included practice and policy guidelines for the aged care sector, as well as work with the Disability Royal Commission on recommendations for supported decision-making. Practice tools derived from both of these projects will be shared as part of the talk, along with experiences in implementing these projects in the home and community care context.

Learning Outcomes

It is intended that registered attendees will:

  • Understand the conceptual and practice principles of advance care planning, and its intersection with supported decision-making
  • Understand the role of meaning-based coping for people ageing with disability and/or life-limiting conditions
  • Be familiar with a ‘roles-place-relationships’ framework for eliciting values from a person’s life experience to assist in identifying goals and priorities for current and future care
  • Be familiar with techniques and practical resources to assist with values clarification for people undertaking advance care planning

About the presenter(s)

Dr. Craig Sinclair is a Senior Research Fellow in the School of Psychology at the University of New South Wales. His key research interests include advance care planning and supported decision-making, among older adults and people with cognitive disabilities. He is currently leading an NHMRC funded clinical trial of reminiscence-based life story work and advance care planning, among older and culturally diverse adults receiving home care services in Australia.
